The Support at Home program features eight service categories. Each classification has an assigned funding level for you to access the services you need.

You also have access to short-term funding pathways under Support at Home.

  1. Restorative Care Pathway: This pathway will expand upon existing arrangements under the Short-Term Restorative Care (STRC) Program by increasing support from eight (8) weeks to sixteen (16) weeks. You can utilise this pathway to maintain and improve your independence, delay reliance on additional services, and continue participating in activities you enjoy. This pathway will assist you to reach your renablement journey through increased clinical supports such as nursing and allied health specialists.
  2. End-of-Life Pathway: This pathway provides access to additional services in the last three (3) months of life for Australians who prefer to remain living at home during that time.

To further support your needs, the AT-HM funding scheme will provide access to Assistive Technology and Home Modifications without requiring you to draw on funds from your Support at Home quarterly budgets.

Assistive Technology includes items, aids, and equipment or products that help you do things more easily or live more independently.

Home Modifications enable changes to your home to make it safer and more accessible for you to live the way you want as you age.

Transitioning to Support at Home from a Home Care Package

The Support at Home program automatically transitioned existing Home Care Package (HCP) recipients from November 1, 2025.  The transition is automatic, and we will work with you to understand new features, including potential cost-sharing, separate Assistive Technology funding, and a new Integrated Assessment Tool for future reassessments, to ensure you receive the best personalised care and equipment. 

You will be allocated a budget aligned with your Home Care Package level (or the level you have been approved for and are awaiting access to).

If your needs increase, we can support you in obtaining a reassessment through My Aged Care to a new Support at Home classification with a higher budget.
