What is the Support at Home Program?

The Australian Government has introduced the Support at Home program to simplify and enhance the delivery of home care services.

The new program replaced Home Care Packages and the Short-Term Restorative Care Programme on 1 November, 2025.

Under the Support at Home Program there will be improved access to services, products, equipment and home modifications to help older Australians to remain healthy, active and socially connected to their community.

What do the changes aim to do?

• Easier and faster access: Reducing wait times for care services and simplifying how you access them.
• Additional funding tiers and pathways: More options for funding things like home modifications to improve your independence and comfort.
• Enhanced personalisation: Care plans that are better tailored to fit your unique health needs and lifestyle preferences.

What does this mean for you?

If you are currently on a Home Care Package, you will keep your current level of funding and unspent funds. You won’t pay any more and you might pay even less. You will be able to access funding up front for specific goals like assistive technology and home modifications. Your Care Partner can explain all of the new features of Support at Home program and how they affect you.

Funding classifications and pathways

The Support at Home program features eight classifications for ongoing services. Each classification has an assigned funding level for you to access the services you need.

Our classification and associated budget will be determined by an assessment and based on your needs. For more information on assessments, please click here.

You also have access to short-term funding pathways under the Support at Home program.

1. Restorative Care Pathway: This pathway will expand upon existing arrangements under the Short-Term Restorative Care (STRC) Programme, which allows you funding to focus on reablement for 12 weeks.  You can utilise this pathway to maintain and improve your independence, delay reliance on additional services, and continue participating in activities you enjoy.
2. End-of-Life Pathway: This pathway provides access to additional services for up to 16 weeks for Australians who prefer to remain living at home during that time.

To further support your needs, the AT-HM funding scheme will give you access to Assistive Technology and Home Modifications without needing to save up funds from your Support at Home quarterly budgets.

Assistive Technology includes items, aids and pieces of equipment or products that are used to help you to do things more easily or live more independently.

Home modifications enable changes to your home to make it safer and more accessible for you to live the way you want as you age.

Transitioning to Support at Home from a Home Care Package

If you currently access services via a Home Care Package or you’re currently waiting to be assigned one on the National Prioritisation System, you will not be automatically reassessed into one of the new classifications when Support at Home starts on November 1, 2025.

You will be allocated a budget that aligns to your Home Care Package level (or the level you have been approved for and are waiting to access).

If your needs increase we can help you be reassessed into a new Support at Home classification with a higher budget.
