Ozcare can help guide you through the assessment process with My Aged Care to start a Home Care Package. Once you are assigned a package, you will be given a dedicated Care Manager to support you in selecting a range of services and supports to help you live your best life.

What can I spend my Home Care Package on?

Home Care Packages are designed to offer support and assistance to Australians over the age of 65 who require help with daily tasks. Funded by the Australian Government, these packages support seniors to live independently and comfortably at home for as long as possible.

How our Home Care Packages work

Eligibility for Home Care Packages is subject to an assessment completed by the Aged Care Assessment Team via My Aged Care. Depending on your needs, you will be approved for a Home Care Package (HCP) level ranging from 1 to 4. Once approved, you will be placed on the national waitlist. You will receive notification when you have been assigned a package. In some cases, the level of package assigned to you may be lower than what you have been approved. It is your choice whether you accept the assigned package or you can remain on current services waiting for assignment of your approved level of care.

The package levels are:
- Home Care Package level 1
Supports people with basic care needs - Home Care Package level 2
Supports people with low level care needs - Home Care Package level 3
Supports people with intermediate care needs - Home Care Package level 4

With an Ozcare Home Care Package, you have the flexibility to manage and direct your own care support and services. You will also receive an individualised budget so you can see what funding is available and how those funds are spent.

Steps to Apply for Home Care

Home care services are designed to help you stay at home for longer, and can even assist you to become more active and social.
Getting home care can be done in six easy steps:
Step 1. Register with My Aged Care on 1800 200 422 (Have your Medicare card ready)
Step 2. Assessment – My Aged Care will refer you for services straight away if your needs are very basic, or will arrange for you to have an assessment
Step 3. Submit your Home Care Package Calculation of your cost of care form (SA456) (For Home Care Packages only)
Step 4. Find a Provider – Find your nearest Ozcare branch or ask My Aged Care to refer you to Ozcare
Step 5. Review Your Service Agreement & Costs – Sign your service agreement, discuss the costs, and create your individual care plan. If you are on a home care package, you will also create your personalised budget
Step 6. Start Services – You’ll start receiving your services from our experienced team of carers, nurses, dementia specialists, and allied health professionals such as physiotherapists
