Applying for Support at Home
Helping you access the services you need

If you are already receiving services
If you are currently receiving services via a Home Care Package, you will automatically transition to Support at Home with no action required on your behalf.
If you have been assessed and are awaiting allocation of a Home Care Package, you will transition to the Support at Home program once a place becomes available.
When you enter the Support at Home program, you will be allocated a budget aligned with your Home Care Package level (or the level you have been approved for and are awaiting access to).
You won’t need a new assessment unless your needs change. If you need more support, please get in touch with your Care Partner, who can guide you through this process.
If you are new to supported aged care
If you don’t currently receive aged care services, but think you may need support, you don’t need to wait. We can help you navigate the assessment system now. Just give us a call on 1800 692 273.
The Single Assessment System for aged care has been developed to simplify and improve the assessment process, making it easier for older people to access government-funded aged care. You will be supported throughout the process to access services.
The assessment process:
- Register online through My Aged Care or call 1800 200 422
- Complete an eligibility check and provide information on your needs
- Undergo an in-person assessment in your home and develop your care plan with your aged care assessor based on your goals and needs
- Contact Services Australia to complete your income and asset assessment
- Select Ozcare as your provider
- Your local Ozcare service will contact you
- Begin receiving services.
Once you’ve been assessed
Following assessment and acceptance into the Support at Home program, you will receive a Notice of Decision and a support plan.
This will contain:
- A support plan that itemises your approved services with a summary of your goals and needs
- A classification with an associated ongoing quarterly budget; and/or
- An approval for short-term support, which may include a budget for:

If you are assessed to receive an ongoing quarterly budget classification, you will have access to an approved list of services. Working with your independent assessor, you can determine the services from that list that you wish to receive.
