From advice about transitioning to aged care or managing diabetes through to installing hand rails in your home or treating a shoulder injury, together we’ll create a personalised care plan that takes into account your specific needs.  The experienced team at Ozcare provides a suite of services that improve your mobility and well-being.

Assessment for Mobility, Environment and Home Modification

If you’re experiencing difficulty keeping your balance or moving around your home, our allied health team can help you find the best ways to keep you safe and mobile.

This can include:

  • Adding hand rails or making safety modifications to your home
  • Attending group programs such as Find Your Feet to prevent falls and improve balance
  • Referrals for the use of equipment and aids such as ramps, toilet aids, shower chairs and wheelie walkers
Assessment for Aids and Equipment

Our team of health professionals conduct assessments in your home and community to advise you about different products that can help you remain independent in your home.

We provide you with a referral and in some cases help you to access state government funding for subsided aids and equipment. Read more about the Queensland Government’s medical aids funding programs.

The aids and equipment we refer include:

  • Mobility aids such as wheelchairs and wheelie walkers
  • Daily living aids such as hoists and shower chairs
  • Continence aids
Social Work

If you need help with adjusting to changes in your life, or are having difficulties with illness, injury or relationships, our social workers can help you with education, advice and support.

There are many reasons why you might want to speak with a social worker:

  • Adjusting to a recently diagnosed illness, treatment or injury
  • End of life support such as dealing with grief or concerns about death or dying
  • Carer issues such as stress, burden or crisis
  • Elder abuse including self-neglect
  • Family conflict that interferes with medical treatment or recovery
  • Transition into aged care facilities including the application process
  • Financial concerns, mental health and legal issues
Diabetes and Weight Management

Our team of health professionals can help you with the challenges of living with diabetes such as maintaining a healthy weight and keeping health problems at bay. We work with you to find the best solutions and methods of support that you can make part of your everyday life.

Pain Management and Symptom Control

Our Podiatrists are qualified health professionals specialising in the prevention, diagnosis, treatment and rehabilitation of disorders, medical and surgical conditions of the feet and lower limbs.

Services include:

  • Foot checks to test foot mobility, sensitivity and injury
